Paperwork handled with care
Forms have a way of piling up exactly when you have the least energy for them. Disability and leave paperwork, school and employer accommodation forms, insurance documentation, letters of medical necessity, records requests, letters supporting gender affirming care, and advocacy letters. We handle these as part of your care rather than as an afterthought.
Requests are reviewed by your clinician, not rubber stamped by an office. If something needs a conversation before it can be written honestly, we will have that conversation with you first.
Your privacy is not negotiable
Because we do not bill insurance, we are not obligated to hand your records to third parties. Nothing goes to an insurer, an employer, or anyone else unless you specifically ask us to send it.
For a lot of patients, this is the whole reason they came here. What you talk about in this office stays in this office, and you decide what leaves it and where it goes.

What kinds of letters can you write?
Common requests include disability and leave paperwork, school and workplace accommodation forms, letters of medical necessity, letters supporting gender affirming care, insurance documentation, and advocacy letters. If you are not sure whether something fits, ask and we will tell you.
How long does a request take?
Most requests are turned around promptly, though timing depends on complexity and whether a visit is needed first. Give us as much lead time as you can, especially for anything with a deadline attached.
Will my records be shared with my insurance company or employer?
Not unless you ask us to send them. Because we do not bill third parties, we are not required to disclose your records to insurers, and we hold to strict standards of privacy.
Is there a charge for documentation?
Documentation support is part of your care as a member. For non members, and for more involved requests, contact the practice and we will tell you what to expect before any work begins.
